    1. The patient refuses.
    2. The patient is neutropenic with a fever and the information to be gained is subordinate to the risk.
    3. A more capable examiner just did one.
    4. It is not relevant to their office visit for asthma.
    5. The patient is in a neck immobilizer and the risk of cord damage from turning exceeds the information to be obtained from the exam.
    6. The lady just had a colonoscopy yesterday.
